and Wellness Program and our experienced Lifestyle Coaches,

1. LIFESTYLE CHANGE COACHING: Assessment and
Goal Setting for getting on track.
This program includes:
Fill out and submit the online form on the counseling
page on this site (click counseling form).
Your Lifestyle Change Coach will review the form
and your goals.
Questions and answers, goals setting, change education
and tools are addressed.
Change plan is finalized and implemented.

2. ONGOING FOLLOW UP AND SUPPORT
Support during this critical stage in the program
ensures that you stay motivated, positive and committed.
Online support is provided in three 15-minute follow-ups
over the next 3 weeks.
We will review your progress and ensure that we help
you to implement new behaviours, help you stay you on
track and make any adjustments required to reach your
weight loss goals!
Includes:
Online support in three 15 minute follow up sessions
over three weeks.
A review of your progress
Support, direction and education during the program
Ideas and exercises for implementing new behaviours
to help keep you on track.
For best results, the ongoing follow up and support
program is most effective when used with the tools acquired
through the original Lifesyle Change Coaching program.
The ongoing follow-up support is available as a stand-alone
support mechanism but does not include a comprehensive
assessment and plan.

3. COMPREHENSIVE “CHANGE FOR SUCCESS”
PROGRAM
Over four weeks, this program encompasses the two
programs above and incorporates ”best practices”
in lifestyle change counseling and support.
Includes :
Assessment, goals, strategy and plan (first week)
Ongoing Lifestyle Change support
Feedback and ideas about staying focused and committed
Tips and tools to stay on track
Review of your progress to date
Motivation and inspiration tactics
Three 15 minute follow-up sessions in weeks 2,3
and 4.
With the assessment, planning, support, follow up and
tips and tricks provided by your personal change coach,
you’ll be well on your way to realizing the success,
clarity of purpose and personal freedoms you’ve
